Medicare Part A (Hospital Insurance) Costs
​Monthly premium:

  • Usually $0 if you or your spouse paid Medicare taxes while working for a certain amount of time (usually 10 years or 40 work quarters). If you get Medicare earlier than 65, you won’t pay a Part A premium. This is sometimes called “premium-free Part A.”
  • If you don’t qualify for premium-free Part A, you might be able to buy it. Each month, you’ll pay a premium of either:
    • $278 if you paid Medicare taxes for 30‒39 work quarters
    • $518 if you paid Medicare taxes for less than 30 work quarters
Part A late enrollment penalty: If you don’t buy it when you’re first eligible for Medicare (usually when you turn 65), your monthly premium may go up 10%. You’ll have to pay the penalty for twice the number of years you didn’t sign up.
NOTE: Original Medicare will be billed for your hospice care, even if you’re in a Medicare Advantage Plan. When you get hospice care, your Medicare Advantage Plan can still cover services that aren’t part of your terminal illness or any conditions related to your terminal illness. ​Medicare Part B (Medical Insurance) Costs

Monthly premium: The standard Part B premium amount in 2025 is $185.00. Most people pay the standard Part B premium amount. If your modified adjusted gross income as reported on your Internal Revenue Service (IRS) tax return from 2 years ago is above a certain amount, you may pay an Income Related Monthly Adjustment Amount (IRMAA) as shown on page 4.

Late enrollment penalty: In most cases, if you don't sign up for Part B when you're first eligible, you have to pay a late enrollment penalty for as long as you have Part B. Your monthly Part B premium may go up 10% for each full 12 months in the period that you could’ve had Part B, but didn’t sign up. You may also pay a higher premium depending on your income. Also, you may have to wait until the General Enrollment Period (from January 1–March 31) to enroll in Part B. Coverage starts the month after you sign up.

​What is IRMAA?  Why should this Concern Me?


IRMAA is an extra charge added to your premium. The total Part B premiums for 2025 are shown below.
Medicare beneficiaries who earn over $106,000 a year and who are enrolled in Medicare Part B and/or Medicare Part D – pay the income-related monthly adjusted amount (IRMAA), which is a surcharge added to the Part B and Part D premiums.

IRMAA was created in 2003 to help ensure the financial stability of Medicare. It initially only applied to Part B premiums, but the Affordable Care Act expanded it to include Part D as well. 

Since 2007, high-income Medicare enrollees have been required to pay the IRMAA surcharge for Medicare Part B coverage. An IRMAA surcharge for Medicare Part D premiums took effect in 2011.
For Medicare beneficiaries who receive Social Security retirement benefits, the premium for Part B is deducted from their Social Security check. This is true regardless of whether the person is subject to the IRMAA surcharge. But the “hold harmless” provision that prevents Social Security checks from decreasing from one year to the next does not apply to people who pay the IRMAA surcharge.
(The “hold harmless” provision has not been applicable in recent years, due to fairly large cost-of-living adjustments (COLA) for Social Security benefits. The 2023 COLA was historically large, and Part B premiums declined for 2023, including the Part B premiums that are paid by people subject to the IRMAA surcharge. For 2024, the average Social Security retiree benefit increased by $59/month, and standard Part B premiums increased by just under $10/month. For 2025, the average Social Security retirement benefit is increasing by about $50/month, and the Part B premium is projected to increase by about $10/month. So the hold harmless provision continues to be non-applicable in nearly all cases.)
IRMAA stands for Income-Related Monthly Adjustment Amount, which is a surcharge added to Medicare Part B and Part D premiums for people with higher incomes. The Social Security Administration (SSA) determines if a beneficiary owes IRMAA based on their income tax return from two years prior. The SSA uses a sliding scale based on modified adjusted gross income (MAGI) to calculate the IRMAA amount.
  
Benefits can request a lower IRMAA if they've experienced a life-changing event that reduced their income, such as: marriage, divorce, death of a spouse, loss of income, and employer settlement payment.  
Benefits can also appeal an IRMAA decision if they believe there's an error in the tax information used to calculate it.
